    QR code registration form template

A minimal starting point: who arrived, consent, and how they paid.

A QR registration form replaces the paper register at your entrance. You print a poster with a QR code; visitors scan it with their phone camera and fill the form in the browser — no app — and your staff see each registration at the counter the moment it is submitted.

Start from the general template below, or from one built for a specific kind of venue. Each registration gets a number, a confirmation the visitor can show at the desk, and a check-in record.

    How to use it

    1. 1

Pick a template and edit the fields

Add the questions your venue needs, set participant details to required, optional or hidden, and choose which consents must be ticked.

    2. 2

Publish and print the QR poster

Publishing activates the form’s QR code. Create one QR per entrance or counter; each registration records which one it came through.

    3. 3

Visitors register on their own phone

They scan, fill the form and submit. The confirmation screen shows their registration number.

    4. 4

Check them in at the counter

Staff search by registration number, name or mobile, take payment by cash, UPI or another counter method if you charge entry, and check the visitor in — and out, if you track who is still inside.
