About this template
Most new hires decide in their first few weeks whether a job is going to work out, long before they say anything about it. A new hire onboarding survey asks them early, while it is still easy to fix: did their laptop and access work on day one, are expectations clear, do they know who to ask for help, and does the role match what they were told when they were hired. The answers show you where your onboarding process breaks, not just how one person is doing.
Employee feedback templates run from onboarding through engagement pulses to the exit interview. They work best when people believe the answers are anonymous, so keep the question count low, avoid anything that identifies a small team, and publish what you changed as a result — response rates on the second round depend almost entirely on what happened after the first. Everything in this template is editable — treat the questions below as a starting point that already has the structure right, then cut what does not apply to you.
- Who should use it
- HR teams, people managers and founders onboarding new employees, in an office, on a shop floor or fully remote.
- When to use it
- Send it at the end of the first week, and repeat it at 30, 60 and 90 days to see how the picture changes.
What this template helps you measure
- Preferred dates and scheduling
- Staff performance
- A clean split you can segment every other answer by
- Scored ratings you can trend over time
- Preferences across a fixed set of options
- Open-ended feedback in respondents’ own words
Every question in this template
8 questions, with what each one is doing there. Change or remove any of them after you make your copy.
Did you have everything you needed on day one (equipment, system access, a clear schedule)?*
Yes / NoWhy this matters: Forces a decision about preferred dates and timing rather than a comfortable middle option, which is the point of asking it this way.
Was it clear who to go to with questions during your first few days?*
Yes / NoWhy this matters: A clean yes or no — nothing to interpret, and it doubles as a filter you can split every other answer by.
How would you describe the welcome you received from your team?*
Star ratingWhy this matters: Gives you a baseline for how your people performed — the number that makes the next round of answers mean something.
Did your manager clearly explain what's expected of you in your first 30, 60, and 90 days?*
Yes / NoWhy this matters: Two answers means two segments, and segments are what make the rest of the report readable.
How does the actual job compare to what you expected based on the interview process?*
Single choice- Better than expected
- About what I expected
- Somewhat different
- Very different
Why this matters: Fixed options keep answers comparable, so one reason can clearly out-rank the others instead of arriving as fifty different phrasings of the same thing.
Do you feel like you understand what success looks like in your role?
Star ratingWhy this matters: Turns a subjective impression into a score you can trend over time and compare across teams or locations.
How comfortable are you raising a question or concern with your manager?
Star ratingWhy this matters: A scored answer is the part of this template you can chart — the open questions explain the movement, this one detects it.
What's one thing that would make your first few weeks easier?
Long textWhy this matters: Unprompted wording surfaces problems you did not think to ask about — the ones no fixed-option question in this template would have caught.
